ony temptacyon / decepcyo~ or variacyon
comynge by sorowe peyne of
sykenesse: or by ony feblenesse of body /
or by ony other occasyo~ what som
euer it be / I fall or declyne i~ peryll of

O My souerayne lorde Ihesuthe veray sone of almyghty god
and of the moost clene and gloryous
vyrgyn Mary / that suffred the bytter
deth for my sake and all mankynde
vpon good fryday & rose agayne the
thyrde daye. I beseche the lorde haue
mercy vpon me that am a wreched
synner but yet thy creature. And for
thy precyous passyon saue me and
kepe me from all perylles bothe bodyly
and ghostly / and specyally from
all thynges that myght tourne thy
dyspleasure / and with all my herte I
tanke the moost mercyfull lorde for
thy grete mercyes that thou hast shewed
me in the greate daungers that
I haue ben in / as well in my soule / as
in my body. And that thy grace and endles
mercy hath euer kept me / spared
me and saued me from the houre of
my byrthe vnto thys tyme. I beseche
the lorde that thy mercy may kepe me
for the alwaye / and I crye the mercy

with all my hole herte for my greteoffences / for my grete vnkyndenesse
and for all my wreched and synfull lyfe /
and that I can not lede my lyfe as
thy seruaunt I crye the mercy.

many other creatures whiche haue
a thousande tymes deserued better

illud dulcissimum corpus tuum quod
ex ea sumpsisti: exaudi preces meas et

libera me ab omni tribulatione et angustia /
et a morte subitanea / et ab omnibus
insidijs / et ab omnibus michinocentibus /
